Our responsibilities
Participating covered healthcare providers must protect the privacy and security of protected health information, provide notice of their legal duties and notify affected individuals when required after a breach of unsecured information.
Permitted uses and disclosures
Health information may be used or shared for treatment, payment and healthcare operations, and when otherwise permitted or required by law. Uses outside those purposes may require written authorization.
Your rights
Depending on applicable law and the responsible clinical entity, patients may request access, copies, corrections, restrictions, confidential communications and an accounting of certain disclosures. Patients may also file a privacy complaint without retaliation.
Contact and complaints
Privacy questions may be sent to info@rejuvonix.com without including medical details. Final privacy-officer contact information and complaint procedures will be published before clinical intake begins.
Pre-launch limitation
This website currently does not accept real clinical submissions. This draft does not replace the notice issued by an independent provider, medical group, pharmacy or other covered entity.
